Internship Program Evaluation Forms
for Interns and Supervisors
An
evaluation form is required from all interns and supervisors participating
in the 2001 Los Angeles County Arts Internship Program. The forms are
Microsoft Word compatible. You can download and fill out the form on your
computer. You may return it by fax, e-mail, or regular mail. If you are
unable to download the form, you may request a hard copy.
